The common causes are stale material weighted to the pre-2026 paper, running out of time, treating Federal Requirements as the small domain, and answering federal questions from state practice.
PTCB publishes no pass rate, so nobody can tell you how many fail. What is visible is why, and the reasons repeat.
One: the material was out of date
The outline changed on 6 January 2026. Federal Requirements went from 10 scored questions to 15, Medications from 32 to 28.
A candidate revising from older material spends a third too long on drug names and a third too little on federal rules, and does so diligently. Ask which one it was.
Two: they ran out of time
Ninety questions in 110 minutes, with roughly a fifth of the scored paper being arithmetic. Candidates who never sat a full-length timed paper discover the pace on the day. Four responses, not one.
A candidate who ran out of time and responds by learning more content will run out again. The fix is fluency in conversions, a two-pass approach, and having sat the full length once. Diagnosing which failure you had is more important than reacting to it.
Three: they skimmed Federal Requirements
It is the smallest domain at 15 scored questions, so it looks skippable. It is also the most learnable, because the answers are published federal regulation.
Skipping the domain with the highest marks-per-hour is the most expensive decision available on this paper.
Four: they answered from their own pharmacy
Many state rules are stricter than the federal ones, and a technician answers from daily practice without noticing. The federal position is what is examined.
This is a specific and avoidable failure and it disproportionately affects experienced candidates.
What is rarely the cause
Ability. The material is not conceptually difficult, and almost nobody fails this exam because they could not understand it.
The diagnostic question
After a fail, wait for the scaled score and ask which of the four it was. They need four different responses and only the first two look similar from the outside. Diligently, and wrongly.
