Log into the account you used to register and download it again - most providers keep certificates available for the full five years. If the account is lost, contact the provider with your name, date of birth and approximate exam date.

The easy version
Log into the provider account you registered with. Most keep the certificate downloadable for its full validity period.
Download it, save it somewhere permanent this time, and email a copy to yourself so it survives the next phone.
If you cannot get into the account
Contact the provider directly. They will normally verify you with your name, date of birth, and roughly when and where you sat it.
Some charge a small reissue fee. It is far less than a retake.
If you do not remember the provider
- Ask your employer at the time - they kept a copy for the health department file.
- Search your email for the registration confirmation.
- Check your bank statement from around that date.
- Ask your local health department, which may hold a record of the certificate on the establishment file.
The health department route is the one people forget, and it works surprisingly often for a certificate that was registered against a permitted establishment.
If the employer holds the only copy
The certificate is yours, not theirs. You are entitled to a copy, and a former employer refusing one is an inconvenience rather than a barrier - the provider can reissue it.

- Note the expiry date in a calendar with a reminder three months before.
